Position Summary:

Responsible for facilitating all patient discharges and transfers from the Inpatient Unit. The Discharge Planner functions as coordinator of discharge plans, collaborating with the patient and caregiver, the patient's care team, outside facilities and DME vendors, to ensure safe and timely patient discharges and transfers.

Position Requirements:

• Bachelors of Social Work with Masters preferred from a program accredited by the Council on Social Work Education (CSWE).
• Active member of the National Association of Social Workers or Association of Oncology Social Work preferred.
• 5 years Medical Social Work experience with 3 years Oncology experience/knowledge.
