Job Description:

Under the direction of the Director of Quality and Risk Management, contributes to the organization’s quality management, performance improvement and accreditation readiness activities. Works with the Director, leaders, the medical staff and stakeholders, to develop and implement processes focused on the improvement of patient outcomes and healthcare delivery. Act in role of patient advocate as needed, communicates patients’ questions, complaints, and concerns to the appropriate staff member and guides resolution. The advocate may be required to review policies and procedures to patients and refer them to proper services. Advocates may be assigned additional responsibilities within scope of practice.

Job Responsibilities:

  • Performs collection, compilation, analysis, and reporting of assigned quality metrics.

  • Makes recommendations for improvements for unacceptable variances in metric performance.

  • Abstracts and submits data to The Joint Commission /Centers for Medicare and Medicaid Services for assigned metrics.

  • Abstracts, audits, and submits data from the Electronic Health Records, EHR to meet scheduled timeframes for Enterprise-wide or facility specific quality dashboards.   Completes an analysis of data and works with leaders to provide action plans to address variances

  • Maintains schedule of reports needed to support various organization and medical staff committees, including Quality Patient Safety Committee, MEC, Medicine Department and Surgery Department meetings. 

  • Completes assigned functions to support additional organization committees including, but not limited to:  Commission on Cancer, Infection Control, Credentialing, Utilization Review, and Peer Review.

  • Supports the Director and Medical Staff leaders in peer review activities.  Completes assigned duties in a timely manner.

  • Prepares analytical charts and reports for presentation to the organization’s leaders as assigned by the Director.

  • Collaborates with leaders and the medical staff to identify areas for improvement and development of action plans for implementation.

  • Reviews medical records to identify areas of concern requiring action by the Risk Manager and/or need for possible peer review.  Reports potential peer review records to the Director and as assigned, to the Medical Staff leaders.

  • Facilitates teams to accomplish quality and patient safety goals.

  • Participates in the organization’s accreditation readiness activities.

  • Documents, investigates, and take steps to resolve patient complaints in a timely manner

  • Documents, investigates, and escalates patient grievances in a timely manner

  • Solicits patient feedback via patient rounding and focus groups

  • Works with all internal partners to maintain a high outpatient and inpatient satisfaction score.

Job Requirements:

  • Minimum of 3 years current work experience in an acute care setting

  • Prior experience in record review, data collection, analysis and reporting preferred. Patient Advocate experience is preferred.

  • ?Bachelor of Science of Nursing

  • Must be licensed in the State of Arizona or can obtain licensure by time of hire.

  • Strong communication skills, conflict resolution, critical thinking, computer proficiency in Microsoft office products

  • Certified Profession in Healthcare Quality (CPHQ) is preferred

  • LEAN or Six Sigma training preferred
